The policy fine print
7 health insurance details worth checking
The exact terms differ by insurer, product, variant and policy schedule. Use these topics as a review checklist—not as a promise of claim payment.
01
Policy wording
Why claims may be delayed or declined
Non-disclosure, exclusions, waiting periods, missing documents, policy status or a mismatch in submitted information can affect a claim.
Share complete and accurate medical history in the proposal.
Read exclusions and waiting periods before buying.
Keep written records and copies of all submissions.
02
Room entitlement
Room-rent limits and proportionate deductions
Some policies have room-category or room-rent limits. Depending on policy wording, choosing a room above entitlement may affect related expenses too.
Check the allowed room category and daily limit.
Ask the hospital and insurer about likely deductions before planned admission.
Do not assume every policy applies the same rule.
03
Out-of-pocket cost
Co-payment and deductibles
A co-payment is the share of an admissible claim that you must pay. A deductible is the amount you bear before specified cover responds.
Example: 20% co-pay on an admissible ₹10 lakh amount means ₹2 lakh share, subject to policy terms.
Check whether it applies by age, treatment, location or plan.
04
Hospital access
Network hospitals and cashless treatment
Cashless service depends on current network status, policy terms and insurer authorisation. Network lists can change.
Check your preferred hospitals using the insurer's current official list.
Save the insurer helpline and policy number.
In an emergency, prioritise urgent medical care.
05
Bill review
Non-payable and non-medical items
Some consumables, personal items or other charges may not be payable unless the policy or an applicable add-on covers them.
Ask for an itemised bill.
Review the policy's non-medical expense terms and any add-on.
Do not assume a fixed percentage applies to every hospital bill.
06
Waiting periods
Pre-existing diseases and waiting periods
Initial, specified-disease and pre-existing-disease waiting periods vary by policy and may have permitted modifications or options.
Declare medical history accurately.
Confirm the waiting period for each relevant condition in your policy schedule.
Never assume a condition is covered from day one without written policy confirmation.
07
Long-term cover
Sum insured and renewal continuity
Your cover should be reviewed against family needs, local treatment costs, medical inflation and existing employer cover. Missed renewal payments can put continuity at risk.
Review sum insured, restoration benefits, bonus rules and exclusions.
Set renewal reminders and understand the grace-period rules.
Compare options before changing or discontinuing a policy.

Important: Submit documents within the timeline stated by your insurer/policy. Do not rely on generic online deadlines when your policy specifies otherwise.
If there is a claim issue
A calm, documented escalation path
A query, shortfall or denial should be understood from the written communication and the policy terms. Keep each step documented.
01
Read the written decision
Ask for the exact reason, relevant policy clause and item-wise deduction details in writing.
02
Send a structured query
Attach the claim number, policy details, bills and supporting medical records. Keep proof of submission.
03
Use the grievance channel
Follow the insurer's official grievance process. If unresolved, check the current IRDAI Bima Bharosa and Insurance Ombudsman routes.
No advisor can guarantee claim approval. The insurer assesses a claim under the policy terms, medical records and applicable regulations.
Quick answers
Frequently asked questions
Does a cashless card guarantee approval?
No. Cashless treatment is subject to policy coverage, hospital network status, medical necessity, pre-authorisation and the insurer's assessment.
Is every hospital expense covered?
No. Coverage depends on the policy's inclusions, exclusions, limits, waiting periods and applicable conditions.
Can I compare health insurance options?
Yes. Compare cover, exclusions, room limits, co-pay, waiting periods, network hospitals, claim servicing and total premium—not price alone.
What should I share for a policy review?
Start with the policy schedule and wording. Share personal medical or identity documents only through a secure, appropriate channel and only when needed.

01
Policy
Medical history:
Documents: Bill, Report, Signature
Planned Treatment
03
Cashless Claim
Final Approval: Insurer
04
Reimbursement Claim
05
Documents Checklist
Policy Schedule, Health Card और Policy/Claim Number।
Final Itemised Hospital Bill और Payment Receipts।
Limits
08
Escalation
Email/Complaint Number सुरक्षित रखें।
09
Emergency और Planned Admission
Emergency
Planned Admission
Insurer
Hospital
Network Hospital
Documents
10
Discharge
Cover
12
